Rehabilitation and Therapy

Each person who receives rehabilitation services through our rehabilitation center at West Park has a care plan tailored to their situation and goals following a hospitalization, surgery, or acute medical event. The plan is based on an assessment upon admission, your abilities, and your input. You’ll work with an interdisciplinary team, including your physician and our therapists, to maximize your recovery.

Our comprehensive therapy services include physical, occupational, and speech therapy. Throughout your stay, our medical team works with you and your family to provide the tools and education you need to be self-sufficient and successful once you return home.

Inpatient and outpatient care are both available through West Park.

Skilled Nursing/Long-Term Care

Our extended care community accommodates residents that need a greater level of assistance with daily living tasks. When your loved one resides at West Park, you can rest assured he or she will be safe, secure, and cared for by compassionate, qualified professionals. A variety of accommodations are available in our Columbus community, including fully furnished private, semi-private, and deluxe semi-private rooms, as well as suites. All are equipped with half baths and free basic cable services.

Memory Care

Memory care at West Park provides a secure environment that allows each resident to feel safe, independent, and comfortable. Our program offers a unique and effective approach to Alzheimer’s and dementia care.

• A 28-bed unit offering semi-private and private resident rooms
• Consistent staff
• A secure outside courtyard
• A large activities space
• Engaging activities
• A Music and Memory program, certified by the Ohio Department of Aging

Respite Care

Respite care at West Park offers short-term stays that can vary from a few days to a few weeks. We provide furnished respite suites with access to a nursing team 24 hours a day, which includes a personalized plan of care.

Respite-stay guests at West Park receive the same patient-directed care as our residents. This includes a compassionate interdisciplinary team that ensures their individual needs are met as well as access to numerous services, activities, and amenities.

Hospice Care

West Park’s signature end-of-life care allows residents to experience their final days with peace of mind and dignity. Our skilled medical professionals understand that a dignified passing is an elemental part of a good life. They are dedicated to providing first-class care in an environment carefully manicured for reflection, relaxation, and release.
