The rehabilitation team at Evergreen is also skilled in delivering the Rivetus​ Rehabilitation iHeart© Rehabilitation Program, Lymphedema Therapy, VitalStim®, Falls Prevention, and LSVT Big and Loud.

Rivetus also offers rehabilitation services to new patients through our Medicare-certified outpatient rehabilitation agency locations or in the patient’s home.

Skilled Nursing/Long-Term Care

Long-term care is a care option that provides personal care support services to residents who can no longer independently perform activities of daily living.

Evergreen recognizes this important stage of life and honors it with signature long-term care that enables you to continue enjoying life’s pleasures free from worrying about diminishing mobility and autonomy capacities.

Memory Care

At Evergreen, we champion choice and independence backed by personal assistance, allowing you to maximize your potential and live life to the fullest.

We offer a safe and nurturing environment, called Anna’s Place, for our residents. Anna’s Place is a memory care unit specializing in enhancing quality of life in a secured setting. Our specially trained staff work with the residents to promote dignity and choice in their daily lives. We offer meaningful recreational activities like reminisce groups, pet therapy, and musical entertainment.

Our caregivers and staff partner with our residents and families to provide innovative approaches to living with memory impairment, including Alzheimer’s and dementia-related disorders.

While providing a personalized program for our residents, we emphasize the full lives that they have led and focus on the activities and interests they have maintained for many years.

The small size of our memory care environment is crucial to the success of lifestyle enrichment. A variety of stimulating activities are offered daily and are the cornerstone of our memory care community at Evergreen.

Respite Care

Respite care is a short-term care option that provides personal support to those seeking temporary respite to relieve stress, restore energy, and promote health.

Managed Care

We accept various managed care insurance plans, which can help alleviate the financial costs of your care. Managed care insurance plans, such as Medicare Advantage and Medicaid, provide comprehensive coverage for skilled nursing services, rehabilitation therapy, and other medical services.

Evergreen’s team of financial counselors is available to help patients and their families navigate the complexities of insurance coverage and benefits. The staff can also work with insurance providers to ensure patients receive the care they need without worrying about the financial burden.

Respiratory Therapy

Our therapists work diligently to treat the underlying causes of your condition while improving the work of breathing with supplemental oxygen.

For short-term residents, the therapist will develop a short-term treatment plan to continue to wean them down to a lower level of oxygen so they can be discharged safely to the comfort of their homes. We have expertise in successfully capping and decannulating tracheostomy residents.

Long-term residents who have received medical treatment for 30 days or longer due to a chronic illness or medical condition are provided the same clinical expertise and attention from all our staff and therapists.
